Physical Exam Requirement
Southington Board of Education policy requires each student to have a health examination during grade nine to be completed by entry into grade ten. The Health Assessment Record (blue form) should be on file by June 1, 2016, but no later than the first day of school when entering tenth grade.
This examination must include a vision test, hearing test, blood pressure, postural screening, hemoglobin or hematocrit, tuberculin risk assessment, chronic disease assessment, and any immunizations update to be verified by your doctor.
Physicals must be completed within one year prior to the first day of school. No tenth grade student may start school without a complete examination and documentation. If your son/daughter's physical is not on file by the first day of school, he or she will be sent home.

"HUSKY Health is the State of Connecticut’s public health coverage program for eligible children, parents, relative caregivers, senior citizens, individuals with disabilities, adults without children, and pregnant women. Children up to age 19 may qualify, regardless of family income level.
