In order for your child’s health needs to be met in the school environment, please notify the nurse immediately of any illness or injury your child has or any concern you may have about your child's health.

  • Update any address or telephone numbers as they change throughout the school year.
  • The State of Connecticut HUSKY Plan offers a comprehensive health care benefit package for Connecticut children up to age 19. Depending on your family’s income, there may be no cost to you.
  • Notify the school if your child will be absent and indicate reason for absence. Certain communicable diseases may require a physician’s note to return to school. Prior to re-entry these pupils are to be referred to the nurse. Contact your school nurse when your child is absent with a communicable disease.

If your child sustains an injury and is seen by a physician it is important that a note is obtained from MD stating any restrictions (physical education, recess), length of restriction time, crutches, or any accommodations which might be necessary.
