Middle School Sports Requirement:  If your child is interested in trying out and participating in middle school sports they will need a physical examination completed and documented on the State of Connecticut Health Assessment  (blue form) within 13 months of the sport season in which the student will participate.

A second form, the Sports Participation Permission Form, must also be completed and signed by a parent or guardian. These forms are available at the middle schools and on the SPS web site.

Bring the completed forms to the school nurse for review. If all documentation is complete a "yellow card" signed by the nurse will be given to the student to bring to the middle school coach in order to participate in tryouts.

Two dates will be available this summer for your middle school student to obtain a "yellow card" necessary for fall sports tryouts -Wednesday, August 13 and Wednesday, August 20.

Due to abatement projects at the middle schools this summer please go to the following schools on these dates to obtain your yellow cards: J.A. DePaolo students should go to Hatton Elementary School. J.F.Kennedy students should go to South End Elementary School.

SPECIAL NEEDS REGISTRY
The Town of Southington in conjunction with the Commission of Disabilities has established a Special Needs Registry at the Southington Police Department dispatch center to assist residents with special needs in the event of emergency or evacuation. Does Your Child Have Health Insurance? Connecticut’s HUSKY Health offers low-cost or free coverage.

If your child is uninsured and you would like to participate in Connecticut’s HUSKY Health program call the HUSKY information hotline: 1-877-CT-HUSKY (1-877-284-8759), from 8:30 a.m. to 6 p.m. Monday-Friday.You can apply by phone or request an information kit. Or, visit HUSKY at www.huskyhealth.com. Here you can download the application. If you are a parent, relative caregiver or pregnant woman,the application can also be used to apply for adult health coverage. Call your school nurse with questions.
