Please understand that these lists are not requiredfor students to purchase in completion, rather they reflect the supplies that teachers recommend for an easy transition into that course.

Middle School Sports Requirement:  If your child is interested in trying out and participating in middle school sports they will need a physical examination completed and documented on the State of Connecticut Health Assessment  (blue form) within 13 months of the sport season in which the student will participate.

A second form, the Sports Participation Permission Form, must also be completed and signed by a parent or guardian. These forms are available at the middle schools and on the SPS web site.

Bring the completed forms to the school nurse for review. If all documentation is complete a "yellow card" signed by the nurse will be given to the student to bring to the middle school coach in order to participate in tryouts.
