The nurse’s mission is to act as a liaison among students, family, educators and staff. The nurse assesses each student to achieve his/her highest level of academic success and enables each staff member to maintain an optimal level of wellness. Components of nursing service include prevention, assessment, intervention, consultation and evaluation of outcome. This is achieved by utilizing the nursing process in health care promotion, instruction, counseling, screening and referrals and by implementing the procedural aspects of nursing care.

Annual Screening
Vision: Grades K through 6 and 9
Hearing: Grades K through 3, grades 5 and 8
Postural: Grades 5 through 9 except when completed during a physical exam within the same year
Height/Weight: as ordered by MD
Referrals will be sent home for any results that require physician evaluation. Please return referral forms to nurse as indicated.
Pediculosis (Head Lice)
A child with head lice is not permitted in school. The symptoms of head lice include: scratching, white spots (nits), that cling to the hair and sores on the scalp. The louse (crab-like grayish bug) may be seen near the scalp. Treatment with a special medicated shampoo is required before re- entering school. Any child who is found to have head lice will not be permitted back in school until he/she is checked by the school nurse.
Medication
All medication must have authorization forms completed by a physician and parent in order to be administered in school. The forms are located in the school health office and must be renewed annually or when any medication change occurs.
Authorized medications may be administered by a school nurse, RN, LPN, or in their absence a principal or teacher trained in medication administration.
Medications must be in the original labeled or pharmacy prepared container. You must request that the pharmacist prepare two separate bottles if the medication is given at home and at school.
No medication will be accepted in envelopes or baggies. Any type of pills, liquids, creams or lotions should not be sent to school with a child.
A maximum 45-day supply can be brought to the school nurse by a PARENT/ GUARDIAN
All medications must be picked up within one week following termination of order or on the last day of school by parent or guardian.

General
In order for your child’s health needs to be met to the utmost in the school environment, please notify the nurse immediately of any illness or injury your child has or any health concern you may have.
_ Notify the school if your child will be absent and indicate reason for absence.
_ If your child sustains an injury and is seen by a physician, it is important that a note is obtained from the doctor stating any restrictions (physical education, recess), length of restriction time or any accommodations which might be necessary.
_ Return emergency forms as soon as possible and also include your pager and cellular phone numbers if available. Update any address or telephone numbers as they change throughout the school year.
