They are theD.P.T., Polio, Measles, Mumps, Rubella, Hepatitis B and Varicella vaccines. Risk AssessmentA tuberculin risk assessment is required. If no proof is presented to the school nurse then the student will be excluded fromschool. A physical examination is required at the following times: 1). in seventh and eleventhgrade 2). upon entering from anotherdistrict Before entering Kindergarten, a physical exam needs to be done after September 1st of the previous year. No student may start school without proof of the following prior to the first day of school: - blue physical form, completely filled out (parent/guardian fills out the front and the doctor fills out the back, including ahemoglobin/hematocrit, vision, hearing and all standard areas)
- four (4) doses of DTP/DtaP vaccine and three (3) doses of Polio (IPV/OPV) vaccine (note: last dose of DTP/DtaP and poliovaccines must be given after the fourth birthday)
- HIB vaccine up to age five-one (1) dose on or after first birthday
- two doses of MMR vaccine (on or after first birthday)
- three doses (3) of Hepatitis B vaccine (children born after January 1, 1994)
- one (1) dose varicella vaccine on or after first birthday (children born after January 1, 1997)
- A tuberculin risk assessment is recommended.
Annual Screening Vision: Grades K through 6 and 9 Hearing: Grades K through 3, grades 5 and 8 Postural: Grades 5 through 9 except when completed during a physical exam within the sameyear Pediculosis: Grades pre-K through 5 and needed during outbreaks Height/Weight: as ordered by MD Referrals will be sent home for any results which require physician evaluation. Please return referral forms to nurse asindicated. Pediculosis (Head Lice) A child with head lice is not permitted in school. The symptoms of head lice include: scratching, white spots (nits), that clingto the hair and sores on the scalp. The louse (crab-like grayish bug) may be seen near the scalp. Treatment with a special medicated shampoo is required before re-entering school. Any child who isfound to have head lice will not be permitted back in school until he/she is checked by the school nurse. Medication- Authorized medications may be administered by a school nurse, RN, LPN, or in their absence a principal or teacher trained inmedication administration.
- If your child sustains an injury and is seen by a physician, it is important that a note is obtained from the doctor stating anyrestrictions (physical education, recess), length of restriction time or any accommodations which might be necessary.
- Medications must be in the original labeled or pharmacy prepared container. You must request that the pharmacist prepare twoseparate bottles if the medication is given at home and at school.

- No medication will be accepted in envelopes or baggies. Any type of pills, liquids, creams or lotions should not be sent toschool with a child
- A maximum 45-day supply can be brought to the school nurse by a parent, guardian or responsible adult.
- All medications must be picked up within one week following termination of order or on the last day of school by parent,guardian or responsible adult.
GeneralIn order for your child’s health needs to be met to the utmost in the school environment, please notify the nurseimmediately of any illness or injury your child has or any health concern you may have. - Notify the school if your child will be absent and indicate reason for absence.

- Return emergency forms as soon as possible and also include your pager and cellular phone numbers if available. Update anyaddress or telephone numbers as they change throughout the school year.
- Students who have been absent because of chicken pox, impetigo or pediculosis may be readmitted to school with a note from theirparent/legal guardian or physician stating the cause and dates of absence. Prior to re-entry these students are referred to the nurse.
